GENERAL

MATERIAL DESCRIPTION
SERVICES OF A LICENSED PLUMBING CONTRACTOR ON AN AS AND WHEN REQUIRED BASIS PER CONTRACT FOR TWO YEARS.
THE SERVICES REQUIRED PERTAIN TO TOWN-OWNED BUILDINGS AND STRUCTURES, AT THE DISCRETION OF THE TOWN.

TERMS AND CONDITIONS
|
|
|
1. |
All quotations shall be submitted on the forms supplied by the Town of Grand Bay-Westfield.
|
|
2. |
QUOTATIONS SHALL BE IN A SEALED ENVELOPE CLEARLY SHOWING THE QUOTATION NAME AND NUMBER, QUOTATION CLOSING DATE, AND COMPANY’S NAME ON THE FRONT OF THE ENVELOPE. QUOTATIONS SHALL BE SUBMITTED TO:

QUOTATIONS SUBMITTED BY FAX WILL NOT BE ACCEPTED.

Quotations must be received at the Town Office, 609 RIVER VALLEY DRIVE, no later than WEDNESDAY, MARCH 12, 2003 AT 3:00 P.M.
|

4. |
Prices quoted shall be specified as an hourly contracted labour rate and percentage on supplied materials.
|
|
5. |
The successful contractor shall provide service during regular business hours as well as after-hours emergency service.
|
|
6. |
The successful bidder shall be required, upon request of the Town, to provide copies of invoices showing contractor’s cost for materials.
|
|
7. |
Work to be done shall be authorized by the Town Manager’s Office for which a Work Order shall be issued prior to the commencement of the work. The Town will not take responsibility for payment of unauthorized charges. Terms of Payment: 30 Days.

8. |
Contractors who shall be considered eligible to bid must have a practicing and current Contractor’s license to do business in the Province of New Brunswick and have been in business not less than one (1) year, are reputable, and have sufficient staff to respond to the Town’s needs. Contractor must supply in the Tender Documents a copy of a license of a Journeyman Plumber working for the Contractor.

9. |
The Town shall make the final determination as to the acceptability of the bidders.
|
|
10. |
A primary determining factor for award shall be the lowest hourly rate for a Licensed Journeyman Plumber.
|
|
11. |
The Town reserves the right to establish a Second supplier to ensure availability of service in the event the successful bidder is unable to respond to the Town’s requirements in a timely manner.
|
|
12. |
The vendor shall state three (3) references complete with names and phone numbers of contacts.
|
|
13. |
INSURANCE:
The successful vendor shall be required to provide proof of General Liability and Auto Liability Insurance in the amount of $1,000,000.00 for each claim which shall save harmless the Town from all suits and actions for damages and costs to which the Town may be put by reason of injury or the death of persons and damages to property resulting from negligence, carelessness or any other cause in the performance of this work.

Copies of or original documentation of warranties on equipment purchased by the Town through the contractor, or purchased by the contractor, shall be turned over to the Town for record keeping for future possible needs. |

15. |
WORKERS’ COMPENSATION:
The Contractor shall furnish evidence in compliance with all requirements of the Workers’ Compensation Act of the Province of New Brunswick, including payment thereunder.
The Workers’ Compensation Act of New Brunswick requires employers who employ three (3) or more workers at any one time to register with the Workplace Health Safety Compensation Commission (WHSCC), following which the employer is furnished with a certificate which verifies registration in accordance with the Act (Regulation 82-13 under the Workers’ Compensation Act).
The Contractor must comply with all regulations under the Occupational Health and Safety Act, and the Department of Labor Laws.

16. |
OCCUPATIONAL HEALTH and SAFETY
Regulations Under the Occupational Health and Safety Act - Province of New Brunswick
The Act defines the minimum acceptable standard for health and safety to which the employers must comply. A copy of the Act and its Regulations can be obtained by contacting the Occupational Health and Safety Commission (Reference: Regulation 91-191). The Town of Grand Bay-Westfield requires that you comply with these minimum standards.

17. |
The term of the contract shall be for a two (2) year period commencing from the date of award.
|
|
18. |
The Town reserves the right to extend this contract for an additional twelve (12) month period upon mutual consent and agreement between the Town and the Contractor.
|
|
19. |
Invoices shall be directed to the Town of Grand Bay-Westfield, P.O. Box 3001, Grand Bay-Westfield, N.B., E5K 4V3, ATTENTION: Town Treasurer. In addition to the HST Registration Number, vendors are required to submit the amount of HST separately on invoices. IF YOU ARE NOT REGISTERED FOR PURPOSES OF HST THEN YOUR SUBMITTED QUOTE WILL INCLUDE ALL TAXES INCLUDING HST, IF APPLICABLE.

20. |
The lowest of any quotations may not necessarily be accepted.
|
|
21. |
The Town of Grand Bay-Westfield
will perform periodic checks in regard to the successful contractor’s cost
of material pricing. |
|
22. |
A primary determining factor for award shall be the lowest quotation for the Licensed Plumber’s rate for “regular time”. |
